How to refer someone
Send them our way
Have your friend fill out our contact form and put your name under “How did you hear about us?” Or fill out our Refer a Friend form yourself.
Or tell us at their visit
At their first appointment, your friend just lets our staff know you referred them.
Earn your reward
Once they complete a consultation with our doctors and choose a multi-month package, you receive your $100 credit toward services.

How do I get my reward?
You earn your reward once the person you refer becomes a client. They need to complete a consultation with our doctors and choose a multi-month package.
Which products or packages qualify?
Multi-month GLP-1 packages (three months or more) qualify. One-month purchases, B12 and B12/MIC are not eligible. There's absolutely no sales pressure: your friend chooses what's right for them.
How much do I earn?
You get $100 toward services for every friend who chooses a multi-month package (three months or more). There's just one reward, and it's the same for every friend.
How can I use the reward?
Your reward is a credit toward services at Lifestyle Health Solutions. Rewards are not paid out in cash.
Is there a limit?
No. You earn a reward each time you refer a new patient. Only referrals of new patients qualify.
